Certain conditions may disqualify you from receiving permanent makeup procedures. Please review the following list carefully:

Disqualifying Conditions:

• Individuals under the age of 18
• Pregnant or breastfeeding women
• Individuals with extremely thin or severely wrinkled skin
• Individuals undergoing cancer treatment (chemotherapy or radiation)
• Individuals with open wounds
• Individuals taking steroid medications
• Individuals with bacterial or viral infections
• Individuals with transmissible blood-borne diseases (AIDS, HIV, Hepatitis)
• Individuals with known allergies to anesthetics, makeup, colors, tattoo inks, etc.
• Individuals with bleeding disorders (hemophilia) or healing disorders
• Individuals prone to keloid or hypertrophic scarring
• Individuals with active skin diseases (skin cancer, psoriasis, eczema, undiagnosed aches or blisters)
• Individuals currently using or having used Accutane or other steroids within the past year

Conditions Requiring Prior Consultation and Doctor's Clearance:

• Pregnant or breastfeeding women
• Individuals with diabetes or other medical conditions under doctor's care
• Individuals with high blood pressure, heart conditions, glaucoma, or mitral valve disorder
• Individuals taking blood-thinning medications
• Individuals with seborrheic dermatitis

Failure to disclose any of the above conditions or provide a doctor's clearance note may result in appointment cancellation, forfeiture of your deposit.

To ensure your safety and optimal results, we require all clients seeking skin treatments or waxing to review the following list carefully:

**NOTE: We do not wax guests who've used Accutane or Retin-A products in the last 6 months.

Disqualifying Conditions:

• Active Skin Infections: Any current or recent skin infections, including acne breakouts, cold sores, or impetigo.
• Sunburn: Skin that is sunburned or irritated.
• Recent Chemical Peels or Laser Treatments: If you have undergone a chemical peel or laser treatment within the past 4 weeks.
• Active Herpes Simplex Virus (HSV): If you have an active herpes outbreak.
• Skin Cancer: If you have any undiagnosed skin lesions or a history of skin cancer.
• Severe Allergies: Allergies to any of the products or ingredients used in our treatments, including essential oils, plant extracts, or other allergens.

Additional Considerations:

• Please inform your esthetician of any allergies or sensitivities to products or ingredients before your treatment.
• Avoid excessive sun exposure or tanning in the days leading up to your treatment.
• Discontinue using exfoliating products or harsh scrubs for a few days before your treatment.

Failure to disclose any of the above conditions or provide a doctor's clearance note may result in appointment cancellation and forfeiture of your deposit.
